Anthony Wayne Band Boosters

This is where have important information for 2010-2011 Anthony Wayne band students to prepare for the upcoming season.

Band students/parents should have gotten part of "the packet" in the mail: an introductory letter, schedule, band booster information and an emergency medical form.

Here you will find additional information, such as various forms, list of officers and uniform information.

• Emergency Medical Authorization Form (PDF): from anthonywayneschools.org. Note: this is a slightly older version of the form.

• AWBB 2010-2011 Officers & Committee Chairs
• Preparticipation Physical Evaluation form (PDF): from the Ohio High School Athletic Association. Band students usually only need to complete pages 1 and 2. Student athletes can use the same form for athletics and for band.

• Band activities explained (PDF): Band activities and events, arranged chronologically as the school year progresses
• Volunteer form (PDF): Opportunities to contribute to the success of the band in 2010-11
• Uniform fees (PDF): The form that will be filled out at Uniform Fitting, includes the fees for each uniform item

• Absence Excuse Form (PDF): Fill out if you cannot attend a required band event
